everyone has different house rules.. so you go by their rules.. but in a basic casino game of texas holdem.. It is always the 2 people to the left of the dealer that place blinds.. First to the left of the dealer is in the "Small Blind" and bets half of the second player to the left of the dealer.. the "Big Blind". If blinds are 50/100 the small blind bids 50.. next big blind bets 100.. now if going around the table.. the betting get too high for the small blind to stay in.. He can fold with his 50 in the game.

Only the 2 players to the left of the dealer. The first player posts the small blind, and the second player posts twice this (the big blind).

Truth is... usually just the blinds. Although I've played in limited stakes casinos that had only one blind. Also, in tournament play, after a certain level is reached, besides the blinds an ante is often put into effect. (An ante is a forced bet by all players at the table.)
